Most work in the built environment stalls for the same reason. Nobody is certain what is true, the people involved are working from different pictures, and when the job is finished the record of what happened is scattered or lost. The next job then starts from the same place as the last one.

The point is simple: a weak link makes every link after it weaker, so it gets tested first.
